What is Varicocele?
Varicocele is a condition where the veins within the scrotum—the sac that holds the testicles—become enlarged. It occurs when the valves in these veins, which normally regulate blood flow from the testicles to the heart, are weak or damaged, causing blood to pool and veins to dilate.
The exact cause is not fully known but is often linked to anatomical differences or valve defects that allow backflow of blood. Varicoceles are most commonly found on the left side of the scrotum but can occur on both sides.
While varicoceles are often harmless, they can sometimes lead to discomfort or complications. Common symptoms include a dull ache or heaviness in the scrotum, especially after physical activity or prolonged standing. In some cases, they may cause testicular shrinkage (atrophy) or fertility issues due to increased scrotal temperature affecting sperm production.

Varicocele Diagnosis
Diagnosing varicocele typically involves a combination of medical history, physical examination, and, if needed, additional diagnostic tests to determine severity and the best treatment approach.
- Medical History: The doctor will discuss your symptoms, including pain, discomfort, or fertility concerns, and review your overall medical history.
- Physical Examination: The scrotum is examined and palpated while you are standing. Varicoceles often feel like a “bag of worms” above the testicle due to enlarged or twisted veins.
- Valsalva Maneuver: During this technique, you breathe out forcefully while the doctor gently examines the scrotum. This helps detect enlarged veins.

- Scrotal Ultrasound: Uses sound waves to create detailed images of the scrotum, visualize dilated veins, assess blood flow, and rule out other conditions.
- Doppler Ultrasound: Measures the direction and speed of blood flow in the veins to determine the severity of the varicocele.
- Semen Analysis: Recommended if fertility issues are a concern. This evaluates sperm count, motility, and morphology, as varicoceles can affect fertility in some men.
