What Is Rectal Prolapse?

The rectum is the final part of the large intestine, where stool is stored before it is passed out of the body. When it’s time for a bowel movement, muscles help push the stool through the anus.

Rectal prolapse occurs when the rectum slips out of its normal position and protrudes through the anus, especially during or after passing stool. The term “prolapse” simply means that an organ has moved or fallen out of its usual place.

This condition is more commonly seen in women than men and often affects people over the age of 50. It is also more likely in individuals with a history of chronic constipation or long-term diarrhoea.

How Is Rectal Prolapse Diagnosed?

Diagnosing rectal prolapse usually begins with a detailed physical examination, followed by a few specialized tests if needed.

Physical Examination:
The doctor will carefully examine the rectal area and may gently insert a gloved, lubricated finger into the anus to check muscle strength and rectal support. You’ll also be asked about your symptoms and medical history. In some cases, the doctor may ask you to strain or squat, similar to a bowel movement, to better assess the condition.

Based on this initial evaluation, additional tests may be recommended:

  • Anal Electromyography (EMG):
    This test checks how well the muscles and nerves in the rectum are working. Small electrodes are placed inside the rectum, and the procedure usually takes only a few minutes.
  • Colonoscopy:
    A thin, flexible tube with a camera is inserted into the rectum to visually examine the colon and identify any underlying issues.
  • Anorectal Manometry:
    This test measures how the rectum responds to pressure. A small tube with a balloon is inserted into the rectum to assess muscle function.
  • Barium Enema:
    A special liquid containing barium is introduced into the rectum, which helps create clear X-ray images of the lower digestive tract.
  • Defecography:
    This imaging test uses X-rays or MRI scans to observe how the rectum and colon function during a bowel movement.
  • Lower GI Series:
    Similar to a barium enema, this test provides detailed X-ray images of the large intestine.
  • Transit Study:
    In this test, you swallow capsules with tiny markers visible on X-rays. Over several days, doctors track how these markers move through your intestines to evaluate bowel function.

In some cases, additional urological or gynecological evaluations may be suggested to check for pelvic floor weakness or the prolapse of other organs.
