What Is a Pilonidal Sinus?
A pilonidal sinus is a condition where a small cyst, abscess, or tunnel forms in the crease between the buttocks (natal cleft). It is more commonly seen in men and in people who sit for long hours, such as desk workers or drivers.
Pilonidal sinus is generally classified into two types:
Acute Pilonidal Sinus
This is a short-term condition that usually occurs due to an infection. It often presents with pain, swelling, redness, and tenderness near the affected area. With proper treatment, it typically heals and may not return.
Chronic Pilonidal Sinus
This is a long-standing condition that can worsen over time. Symptoms may be more severe and can include the formation of a small opening (sinus) that may discharge pus. Chronic cases are often associated with repeated infections and abscess formation.
Treatment Options
In the early stages, pilonidal sinus can be managed with medications, proper hygiene, and simple home care. However, in more advanced or recurring cases, surgical treatment may be required to remove the sinus tract and prevent future infections.

How is Pilonidal Sinus Diagnosed?
Pilonidal sinus is usually diagnosed by an experienced proctologist through a simple and thorough evaluation. The process typically begins with a discussion about your symptoms, medical history, and any discomfort you may be experiencing.
Physical Examination
The doctor will examine the affected area to check for visible signs such as redness, swelling, tenderness, or the presence of pus or discharge. This is often enough to identify the condition.
Imaging Tests
In some cases, your doctor may suggest imaging tests like an ultrasound or MRI. These help assess the severity of the sinus and detect any deeper infection or complications.
Biopsy (if required)
If there is any uncertainty in diagnosis, a small tissue sample may be taken and sent for laboratory analysis. This helps confirm the condition and rule out other possible issues.
Once the diagnosis is confirmed, your doctor will recommend the most suitable treatment plan based on the severity of the condition.

In some cases, a pilonidal sinus may improve or temporarily settle on its own, especially in the early stages. However, it often has a high chance of recurring if not properly treated.
For long-term relief and to prevent repeated infections, it’s best to consult a doctor and follow the recommended treatment plan.
In some early or mild cases, pilonidal sinus symptoms can be managed with medications, proper hygiene, and home care. These measures may help reduce pain, swelling, and infection temporarily.
However, for complete and long-lasting treatment, especially in recurring or severe cases, surgery is often required to remove the sinus and prevent it from coming back.
Consult a doctor if you have increasing pain while sitting, notice swelling or a lump near the tailbone, or experience foul-smelling discharge. Early treatment helps prevent complications and speeds up recovery.
